Dear Prestonwood Health Patients:,
We regret to inform you that Prestonwood Health will discontinue in-person clinical operations and permanently close our practice effective October 1, 2025. While this news may come as a surprise, we are deeply grateful for the privilege of serving your health needs.
We strongly recommend that you establish care with a new psychiatric provider as soon as possible to avoid
interruptions in your care. The following psychiatric practices are located near us, are providers that we can
specifically recommend, and are in-network with most insurance companies. Of course, you are not limited to
pursuing care with these individuals and are free to transfer your care to whoever you prefer.

Morgan Records Management is the appointed records custodian for Prestonwood Health in Texas. Its patient records have been transferred to us, and you can request copies at any time using the request form on this page.
In accordance with applicable state law, you may be charged a reasonable fee for the copying and transmittal of records. The exact amount depends on the size of your file and your state's regulations.
Most states allow by law up to 30 days to process records requests. Due to the high volume of requests we receive, it can take up to 30 days. We do our best to process all requests sooner, as we know how important they are.
Yes, with proper authorization. Parents or guardians for minors, attorneys with a HIPAA-compliant release form, and others with notarized permission may submit requests on your behalf. The form will guide you through what is required.
