Our office is located at 5300 Democracy Drive, Suite 130, Plano, Texas 75024. We are conveniently located near the intersection of Dallas North Tollway and Sam Rayburn Tollway (SH 121), just south of The Shops at Legacy. Ample free visitor parking is available in front of the clinic, and our office entrance is located just inside the main building entrance.
